By this time you must be excited as you have completed your one year of internship and you are just one step behind to get a permanent registration form Medical council to practice independently as a medical graduate.  Before you begin make sure you carry loads of patience with you as you will need it most especially in our college, where things move slower than snail’s pace. Don’t make up your mind that it might be a lengthy process prior reading this blog. It’s very simple process can be easily completed with little patience and common sense.

OVERVIEW OF DURATION (app. 10-12 days)

• For submission of completely filled documents app. 1-2 days
• Waiting to get all your certificates from college office app. 3-7 days
• Travelling to Bangalore 1 day
• Applying and receiving of documents in RGUHS and KMC app. 1 day.

Step 1: Complete log book at hospital.

      First of all make sure u have done all your postings and got your log book completed with seal and sign. Of all the HOD’s of concern dept. don’t forget about your NO DUE certificate, get it done with your log book simultaneously. Then go to Med. Supp. For his sign. And seal on the log book, make sure that u have removed/torn all the pages which u have to submit in the respective dept.

Step 2: In College.

Documents needed in college.

1.  Copies of Marks card
2.  Photos (at least 5 color and 5 B/W)
3.  Completed Log book
4.  NO DUE Certificate
5.  Approx. Rs. 2500/- for certificates.
6.  Optional – Receipt of caution deposit ( paid at the time of admission )
7.  Filled Application for PDC – form provided by college
8.  Filled Application for Convocation – form provided by college
9.  Copy of provisional KMC registration along with Original

          Take Xerox of all the marks memos/cards of first to final year. Write 2 different applications. In First application, mention the subject for refund of caution deposit and attach a Xerox copy of NO DUE Certificate and receipt of caution deposit if you have a copy. Note receipt copy is optional. IN second application, mention in the subject all the certificates required in order. i.e.

     I.            Internship Completion certificate

   II.            Bonafide Certificate

 III.            Attempt Certificate

 IV.            Certificate regarding the amount paid for PDC and Degree Certificate.

   V.            Any additional certificate if you want (optional)

NOTE:-  Don’t leave any of the above certificates as they will be needed for applying in Bangalore.

        Along with the application letters, submit documents 1 – 9. This process will take almost 3-7 Days.

Step 2.5: waiting for the processing of documents in college for app. 3-7 days.

Step 3: Receiving the certificates.

NOTE: MAKE SURE that your Name, Dates, Reg. no are Correct to the minute detail when receiving the certificates.

Certificates to be received from college.

     I.            Internship Completion certificate

   II.            Bonafide Certificate

 III.            Attempt Certificate

 IV.            Certificate from college regarding the amount paid for PDC and Degree Certificate

   V.            Application form of PDC with seal and sign of principal

 VI.            Application for Convocation with seal and Sign of principal

VII.            Original of Provisional KMC Certificate if submitted earlier

Step 4: Before Going to Bangalore.

       Get 4 sets of Xerox of all the following documents with attestation from Gazetted officer compulsorily, otherwise they won’t accept.

     I.            Internship Completion certificate

   II.           Bonafide Certificate

 III.          Attempt Certificate

 IV.           Certificate from college regarding the amount paid for PDC and Degree Certificate

   V.            Application form of PDC with seal and sign of principal

 VI.           Application for Convocation with seal and Sign of principal

VII.         Original of Provisional KMC Certificate if submitted earlier

VIII.      12^th Marks Card

 IX.            10^th Marks Card

   X.            3 Passport size color photos.

Step 5: In Bangalore

For Permanent KMC registration.

            In Bangalore, first go to Karnataka Medical Council at #70, 2nd Floor, "Vaidyakeeya Bhavana", K.R. Road,H.B. Samaja Road Corner, Basavanagudi, Bangalore at SHARP 10:30 am. map of KMC

          At KMC a lady will be present at the reception who will give you a form for permanent KMC registration. Submit the filled application along with 3 color photos and a copy of the above mentioned attested documents and ORIGINAL Provisional KMC registration certificate. They will ask for originals for cross checking. NOTE: they will ask for Provisional Degree Certificate tell them you will give the Xerox of it while collecting the Permanent KMC Registration Certificate.

          Next rush to RGUHS University at 4^th T block, Jayanagar. It will cost around 35 rupees, but in any case don’t pay the auto driver more than 50 to 60 bucks!KMC to RGUHS route map

   At counter 1 in the Asst. Registrar Office, submit the following.

     I.            Application form of PDC along with

a.       Attested copies of all marks card

b.      Attested copy of internship completion certificate

c.       Attempt certificate ( if they ask)

   II.            Application form of Convocation

a.       Attested copies of all marks card

b.      Attested copy of internship completion certificate

c.       Attempt certificate ( if they ask)

d.      2 passport size B/W photos

e.      Certificate of Fee detail by the college

            At around 2:30pm same day, Collect the Provisional Degree Certificate from the Registrar ( evaluation) office on the 2^nd floor or else at the same place where you have submitted and the permanent Degree will be sent to your permanent address mentioned in the convocation form in about 3 months.

          Next take 2 Xerox copies of the PDC and rush to KMC again preferably before 4 pm as it is open till 5 pm  Submit the Xerox of PDC and Collect Original KMC registration.

CHECK the Names and Dates Mentioned on both the certificates before leaving the offices.

Step 6 – Enjoy – You are now officially a Doctor!!

Roam around in Bangalore and don’t forget to call your parents and inform them!!! definitely this will be one of the happiest moments of your life
